Father-of-2 Tom Mansfield had miscalculated the amount of powder he was meant to use on kitchen scales.

An inquest in Ruthin heard the 29-year-old from Colwyn Bay fell ill straight after drinking his mixture on 5 January 2021.

Reaching a conclusion of misadventure, coroner John Gittins said the cause of death was caffeine toxicity.

Caffeine is used by some gym-goers, with some fitness websites recommending it for improving sports performance in certain doses.

However, experts have warned that when taking powders there is a risk of consuming over the recommended amount.


On Tuesday an inquest heard Mr Mansfield began clutching his chest & complaining his heart was beating fast after consuming the product.

Minutes later, after going to lie down, he began foaming at the mouth. His wife Suzannah alerted neighbors & family & called an ambulance

Paramedics tried to resuscitate him for 45 minutes but was later pronounced dead at Glan Clwyd Hospital in Bodelwyddan, Denbighshire.


The lowest weight measured by Mr Mansfield's scale was more than 6 times the maximum recommended dose


The hearing was told the scales Mr Mansfield had used to measure the powder had a weighing range from 2 to 5,000 grams, whereas he was attempting to weigh a recommended dose of 60-300mg.

It was this miscalculation which led to a massive overdose.

A post-mortem examination showed Mr Mansfield had caffeine levels of 392mg per liter of blood.

Caffeine levels would typically be 2 to 4 milligrams per liter if someone had drunk a cup of filter coffee.

Mr Gittins said it appeared Mr Mansfield had been aiming for the "mid-range of the recommended dose", but "he got his math wrong" & there were "unintended consequences" which had a fatal effect.

NIAMEY, Jan 3 (Reuters) - Police in northern Niger have seized more than 200 kg of cocaine worth around $8.7 million that was being transported in the local mayor’s official truck, two police sources said on Monday.

The mayor & his driver, who were in the vehicle at the time, were arrested with 199 bricks of cocaine at a checkpoint on the road heading north out of the desert trading hub of Agadez on Sunday.

A statement from Niger's Central Office for the Repression of Illicit Drug Trafficking (OCRTIS) confirmed that there had been a record seizure of more than 200 kg of cocaine in Agadez, but did not give details.

West Africa, most often a transit route for illegal drugs on the way from South America to Europe, has registered a series of record busts in recent years.

Senegal seized more than two tons of pure cocaine from a ship off its Atlantic coast in October, & Gambian authorities seized nearly 3 tons last January from a shipment originating in Ecuador. 

After 17 tons of cannabis resin worth around $37 million were seized in Niger last May, Interpol's drug unit coordinator Jan Drapal told Reuters that drug kingpins were sending larger shipments in response to increasing European demand.

(CBS4) – A judge handed STEM School shooter Devon Erickson, 20, a life sentence on Friday in Douglas County Court following emotional testimony from victims, including the parents of the student considered a hero for charging at the gunman. Since Erickson was 18 years old and an adult at the time of the shooting, he faced a mandatory life sentence.

Erickson was convicted in June of all 46 charges against him for his role in the May 7, 2019, STEM School Highlands Ranch shooting which left Kendrick Castillo dead and eight others wounded. Castillo was among the group of students trying to stop the gunmen.

The judge imposed hundreds of additional years in prison in addition to life in prison without the possibility of parole.

Alec McKinney, the second student involved in the shooting, testified on behalf of the prosecution in Erickson’s trial. McKinney is already serving a life sentence. McKinney, who was 16 years old at the time of shooting, could become eligible for parole after about 20 years in prison under a program for juvenile offenders.

Before the judge’s ruling, Erickson displayed little emotion as he sat with a face mask on and a prison jumpsuit on as victims and their families testified in the courtroom. That included STEM School students, teachers and parents.

Nykiah Thomas one of the parents whose child was shot. Before asking the judge to impose the maximum sentence for Erickson, she described her daughter being in Algebra class when the gunmen stormed in and shot her in the lower part of her body.

“What I remember from that day was her calling me and telling me she was shot and she did not know how many times because both of her legs were bleeding,” Thomas said. “The doctor told her that they would not remove the bullet because it was too close to her fibular artery.”

Several students testified that they have suffered from PTSD since the attack. One told the judge she’s sure that Erickson and McKinney’s intention was “to murder hundreds of students.”

Kendrick Castillo’s parents, John and Maria Castillo, talked about how the shooter took everything from them.

“He was my life, my best friend, I loved him more than life itself. If I could trade places with Kendrick, I would because he had so much to live for,” said Maria Castillo. “There are no words that I can describe the way I feel. I have hate, anger, pain, sadness… all emotions mixed. So many sleepless nights, still waiting for Kendrick to come home.”

“No one can really imagine what our pain is like, we get invited to cookouts with our neighbors, and we agree that we are going to go, and then at the last minute we just don’t show up. We try to be so strong, but life is never the same,” said John Castillo.

“I heard the words, ‘Kendrick didn’t make it’ and everything froze, I fell to my knees and I haven’t been the same since,” said student Alison Thompson.

“I know what it’s like to call your mom and you don’t know whether you are going to die or not. I know what it’s like to get phantom pains in your leg, because that’s where you were shot, twice, by someone you thought you knew,” said student Jeff Jones.

Seventeen-year-old Erriyon Knighton finished third in the men’s 200-meter final at the U.S. Olympic track and field trials on Sunday, becoming the youngest American man to make the team since 1964.

One day after finishing first in the semifinals and breaking Usain Bolt’s under-20 record with a time of 19.88 seconds, Knighton finished the final even faster in 19.84, trailing only Noah Lyles (19.74) and Kenny Bednarek (19.78).

Knighton is the youngest American man to compete on the Olympic track and field team since Jim Ryun reached the 1964 Olympics at 17 in the 1,500 meters.

A four-star football prospect at wide receiver, according to the 247Sports Composite Rating, Knighton turned professional as a sprinter and signed a sponsorship deal with Adidas in January.

Bolt won the last three Olympic gold medals in the 200 meters. Shawn Crawford was the last American to win gold at the 2004 Athens Games.

CLEVELAND, Ohio (WOIO) - About 200 children are missing right now in Northeast Ohio.

They may be in danger and they need our help, and a task force is making progress tracking them down.

Operation Safety Net recovered 25 missing children in its first two weeks, and the sting is continuing right now.

The task force, led by U.S. Marshals, is working with local partners to get endangered kids to safety.

“These are kids that have been abused, neglected. Some involved in human trafficking,” said U.S. Marshal Pete Elliott.

Elliott has seen the nightmares many missing children have suffered.

“Sometimes the situations they—they go to, believe it or not, may be better than the situations they left from,” Elliott said.

“We’ve had some cases where the mother and or father, or both, may have been prostituting their own child,” he said.

In the past 20 days, U.S. Marshals have worked with Cleveland, East Cleveland and Newburgh Heights police departments to find just over two dozen missing children between 13 and 18 years old.

One in four of those teens they recovered are victims of human trafficking.

“Some we found in Miami, Fla. We have Bedford, Bedford Heights, West Side, East Side, Akron, Mansfield and so on,” Elliott said.

“We’re trying to do our part. A number of these children have gone to the hospital after we’ve recovered them to get checked out, so again this is something we take very seriously,” Elliott said.

Operation Safety Net is focusing on Cleveland and its surrounding cities, but they’re covering all of northern Ohio too with help from the Northern Ohio Violent Fugitive Task Force.

It’s made up of about 350 men and women from 125 different departments across the state.

“I’ll tell you this, it will be something we’ll be doing every year. This is our first time we have done this, it’s been uncharted territory for us, but we’ve had great success,” Elliott said.

Operation Safety Net will continue for several more weeks.

(CNN) An 11-year-old boy drove 200 miles across South Carolina to meet a stranger he knew only on Snapchat, the Charleston Police Department said.

After three hours of driving, the young boy spotted a police officer, pulled up next to his vehicle and said he was lost, according to police. The child drove from the northwest town of Simpsonville to Charleston.


He told the officer early Monday morning he had taken his brother's car to go to Charleston where he wanted to live with "an unknown male he met on Snapchat," police said.

But when he lost GPS signal on his father's tablet, he couldn't recover the man's address, police said, as Snapchat messages disappear or are deleted after they're read.
The child was able to give the officer his name, his father's name and his cell phone. He was reunited with his family later in the day.


Police have launched an investigation into the unknown male, Charleston spokesman Charles Francis told CNN affiliate WCSC.


"I would love tonight, right now, anybody who's watching this, who is a parent of a child, especially an 11-year-old, to sit down with your 11-year-old, right now, right this moment and have a conversation about what you're doing on social media, the dangers, the benefits and things that as a parent we need to talk about every day," Charleston Police Chief Luther T. Reynolds told CNN affiliate WCIV.

LAS VEGAS (AP) — A gunman perched high on the 32nd floor of a Las Vegas Strip casino unleashed a shower of bullets down on an outdoor country music festival below, killing at least 50 people and wounding more than 200 as tens of thousands of frantic concert-goers screamed and ran for their lives, officials said Monday. It was the deadliest mass shooting in modern U.S. history.

Country music star Jason Aldean was performing Sunday night at the end of the three-day Route 91 Harvest Festival when the gunman opened fire across the street from inside the Mandalay Bay Hotel and Casino. SWAT teams quickly descended on the concert and the casino, and officers used explosives to get into the hotel room where the suspect was inside, authorities said. The gunman died at the scene and was identified by Clark County Sheriff Joseph Lombardo as Stephen Paddock, 64. A motive was not immediately known.

Aldean was in the middle of a song when the shots came rapidly: Pop-pop-pop-pop. Video of the shooting then showed Aldean stopping and the crowd getting quiet as if they were unsure of what had just happened. The gunman paused and then fired another volley of muzzle flashes from the gold glass casino as more victims fell to the ground while others fled in panic. Some said they hid behind concession stands and other crawled under parked cars.

Kodiak Yazzie, 36, said the music stopped temporarily when the first shots began and the tune even started up again before the second round of pops sent the performers ducking for cover and fleeing the stage.

“It was the craziest stuff I’ve ever seen in my entire life,” Yazzie said. “You could hear that the noise was coming from west of us, from Mandalay Bay. You could see a flash- flash- flash- flash.”

Thousands in the crowd fled as the bullets ran rampant. Monique Dumas from British Columbia, Canada, said she was at the concert, six rows from the stage when she thought she heard a bottle breaking, and then a burst of popping sounds that may have been fireworks. She said as she made her way out, it was “organized chaos” as everyone fled. “It took four to five minutes and all that time there was gunfire.”

In addition to Paddock, police said they located a woman who may have been his roommate — Marilou Danley, 62. Lombardo said they believe this was a “lone wolf” attack.

“It’s a devastating time,” Lombardo said.

Police shut down the usually busy Las Vegas Boulevard and authorities across the state and federal ranks converged onto the scene as dozens of ambulances ferried those struck by gunfire. Nearby Interstate 15 and flights at McCarran International Airport were briefly closed. Hospital emergency rooms were jammed with victims delivered by ambulance. Others loaded the wounded into their cars and drove them to hospitals.

Jose Baggett, 31, of Las Vegas, said he and a friend were in the lobby of the Luxor hotel-casino — directly north of the festival — when people began to run, almost like in a stampede. He said people were crying and as he and his friend started walking away minutes later, they encountered police checkpoints where officers were carrying shotguns and assault rifles.

“There were armored personnel vehicles, SWAT vehicles, ambulances, and at least a half-mile of police cars,” Baggett said.

Among those killed were two off-duty police officers who were attending the concert. Two on-duty officers were wounded, including one who underwent surgery and was upgraded to stable condition early Monday, police said.

Hours after the shooting, Aldean posted on Instagram that he and his crew were safe and said the shooting was “beyond horrific.”

“It hurts my heart that this would happen to anyone who was just coming out to enjoy what should have been a fun night,” Aldean said.

President Donald Trump extended condolences to the victims and their families.

In a tweet Monday, Trump offered “My warmest condolences and sympathies to the victims and families of the terrible Las Vegas shooting. God bless you!”

White House spokeswoman Sarah Huckabee Sanders said Trump was “briefed on the horrific tragedy in Las Vegas.”

Sanders said that “we are monitoring the situation closely and offer our full support to state and local officials. All of those affected are in our thoughts and prayers.”

The shooting at the sold-out Route 91 Harvest festival was the deadliest mass shooting in modern U.S. history. Forty-nine people were killed when a gunman opened fire at the Pulse nightclub in Orlando in June 2016.

Sunday’s shooting came more than four months after a suicide bombing at an Ariana Grande concert in Manchester, England, that killed 22 people. Almost 90 people were killed by gunmen inspired by Islamic State at the Bataclan concert hall in Paris during a performance by Eagles of Death Metal in November 2015.

Following a 13-week break, Kendrick Lamar returns to the top of the Billboard 200 albums chart with "DAMN."

Billboard reports that it's the discs fourth week overall at number one. The best performance by an album since "Starboy" from The Weeknd spent five weeks atop the list in earlier this year.

"DAMN" sold just under 47,000 equivalent album units in the week ending Aug. 10, according to Nielsen Music, to soar past it's competition. It sold 603,000 equivalent album units in its first week following an April 14 release.

"DAMN." has since been certified double platinum by the Recording Industry Association of America.

Kendrick Lamar's new critically acclaimed "DAMN." album is performing extremely well commercially. After topping the charts with 603,000 units sold in it's debut week, the disc is still being gobbled up by fans at a rapid rate.

Billboard reports that the album will sit at No. 1 on the Billboard 200 chart for the second consecutive week, with projected sales of 220,000 to 240,000 equivalent album units in the week ending April 27.

At this rate Kendrick will soon be celebrating an album exceeding one million in sales for the third time. Both "Good Kid, M.A.A.D City" and "To Pimp a Butterfly" have been certified Platinum by the Recording Industry Association of America.

His debut project "Section 80" is certified Gold with sales of 500,000 copies.

Another new album release = another number one debut for Drake.

The Young Money Records recording artist's "More Life" playlist is projected by Billboard to move 500,000 to 550,000 equivalent album units in the week ending March 23. 200,000 to 250,000 of those are from traditional album sales. Good enough to earn the top spot on the Billboard 200 chart.

"More Life" is currently only available through digital retail outlets. Expect it to get an extra bump once physical and vinyl copies are released in the coming weeks.

J. Cole's latest album "4 Your Eyez Only" made a huge impact during it's first week out. The project sold 492,000 units. 363,000 of those were from traditional sales, according to Nielsen Music.

Those numbers were good enough to place "4 Your Eyez Only" at No. 1 on the Billboard 200 chart. It's also the third biggest debut album of 2016, trailing only Drake’s "Views" (1.04 million units, 852,000 traditional) and Beyonce's "LEMONADE" (653,000 units, 485,000 traditional).

The beef between Drake and Meek Mill continues to escalate.

On Sunday, August 21, Drake's "Summer Sixteen Tour" pulled into Philadelphia for a concert at the Wells Fargo Center.

During the show the Toronto rapper told performed "Back to Back" and said Meek "did this to his motherfucking self" and wasn't from Philly.

Meanwhile, Meek and his Dreamchasers goons were outside 200 deep, ready for a confrontation.

Meek made and then deleted an Instagram post claiming nothing happened because Drake was escorted away from the venue by police.

"When u run out the back door wit 12 and use the “old man” 2 save you!" Meek wrote. "I’m done, you had Philadelphia swat with you” Ima go head hang up it with these suckas! Had y’all tucked in back there for hrs lol#stillrunning I only came to talk lol."

A couple of the goons posted videos.

"I'm letting y'all niggas know right now we 100-200 deep. Dreamchasers, we gon' press this nigga drake. He disrespectful," one says.

(ESPN) LAS VEGAS -- Interim UFC light heavyweight champion Jon Jones has been pulled from his title unification bout against Daniel Cormier at UFC 200 on Saturday, after the U.S. Anti-Doping Agency informed Jones of a potential doping violation Wednesday.

UFC president Dana White announced the move Wednesday during an impromptu news conference at the MGM Grand.

Jones tested positive for a banned substance in an out-of-competition sample taken June 16 by USADA, said Jeff Novitzky, UFC's vice president of athlete health and performance. USADA administers UFC's anti-drug policy.

"It is important to note that under the UFC anti-doping policy, there's a full, fair legal-review process that is afforded to all athletes before any sanctions are imposed," Novitzky said. Novitzky added that there wasn't time for a "full review before the scheduled bout." White said he has not spoken to Jones yet and that he wasn't aware whether the substance in question was a performance-enhancing drug. A heavyweight fight between Brock Lesnar and Mark Hunt will now headline UFC 200, which takes place Saturday at T-Mobile Arena.

Jones has the right to appeal any positive test. He has never tested positive for performance-enhancing drugs but did test positive for traces of cocaine before a fight against Cormier in January 2015.

Jones also served a suspension from April to October after his involvement in a hit-and-run accident.

He is scheduled to address the media Thursday at 11:30 a.m. ET at the MGM Grand.

Under UFC's anti-doping program, which went into effect last July, a positive test for a performance-enhancing drug carries a standard two-year suspension.

In an interview with ESPN last week, Jones had expressed hope he could turn his legacy into a positive one.

"I want to show the world that you can be down but never out," Jones said. "I want to be a story where someone risked losing so much but ultimately turned everything around. A lot of times you hear these stories about athletes who ruined their career and they go away and no one knows what happened to them or they're bankrupt or they end up in jail. They just ruined a great career. I want to be one of the few stories you hear where I was ruining things but ultimately turned things around and became a hero. That's my vision for the way my story is going to play out."

White said he would like to find Cormier, the defending light heavyweight champion, a new opponent, but that would be difficult because of the timing.

"When you have the biggest, baddest fight card ever assembled, you know, it doesn't sting as bad when you lose a fight," White said. "But it stings real bad for Daniel Cormier. This is devastating to him, his family, and I'd like for him to fight another guy."

Cormier appeared next to White during the news conference.

"I've trained hard and long," Cormier said. "If anybody would fight, I would fight. Why not? I understand the difficult task it would be to find me a fight on two days. I'm willing to fight up, put on some weight, 225, 220 [pounds]. I'll fight. It doesn't matter. I just can't fight a really big guy, because I've been shrinking my body."

Cormier said he would put in the effort and would accept a new fight if UFC could find one that "made sense."

"I've worked really hard to prepare for this," Cormier said. "You take care of what you need to take care of. That's what I did. More than anything, it's really disappointing."

Cormier said having USADA as UFC's partner in the administration of drug testing "changes a lot of things" and that UFC made a conscious decision to bring in USADA.

"They didn't have to do this," Cormier said. "They decided to clean up the sport. You're going to have casualties."

Cormier said he got a bad feeling in the pit of his stomach when White texted him Wednesday, asking him whether he could make it to the news conference.

"This guy always gives me good news," Cormier said of White. "You know, this guy calls you with a seven-figure check -- 'dude, you're rich now' -- you know, he calls you with good news. And the mood didn't feel very good. It was scary. So I came here, and I was actually early. It's probably the first time I've ever been early. I needed to know. But he's usually the good-news guy. And today he had to give me some pretty bad news."

Interim UFC light heavyweight champion Jon Jones is out of UFC 200. His fight against champion Daniel Cormier has been scrapped.

The U.S. Anti-Doping Agency informed Jones yesterday that a June 16, 2016, out-of-competition sample came back positive for a possible banned substance.

The Brock Lesnar vs. Mark Hunt three-round heavyweight fight is now the main event on the card, which will be held on Saturday, July 9, at the T-Mobile Arena in Las Vegas.

Cormier was devastated upon hearing the news. He desperately wanted to avenge his previous loss to Jones.

"I'm very disappointed. I worked really hard to prepare for this," Cormier said at a Wednesday press conference. "I put myself in a position where I felt like I could win this fight. To hear something like this, it's very sad. From a competitive standpoint, from a financial standpoint ... there are a number of ways in which this is disappointing."

One of the most popular fighters and huge pay-per-view draw Brock Lesnar is returning to the Octagon for a match at UFC 200, taking place on July 9, 2016 at T-Mobile Arena in Las Vegas, Nevada.

The announcement was made Saturday, June 5, at UFC 199 during a promotional video for the upcoming huge card.

The Lesnar fight against an opponent to be named later is just the icing on the cake for a stacked card that already boasted Jose Aldo vs Frankie Edgar 2 (interim featherweight championship), Jon Jones vs Daniel Cormier 2 (light heavyweight championship) and Meisha Tate vs Amanda Nunes (women's bantamweight championship).

Lesnar is still under contract with the WWE. The organization issued a statement on his participation at UFC 200.

“Brock Lesnar remains under contract to WWE, however, he has been granted a one-off opportunity to compete at UFC 200. Following this milestone event on July 9, Brock will return to WWE for SummerSlam on Sunday, August 21, live on WWE Network.”

During Lesnar's previous stint in the UFC he won the heavyweight championship in his fourth fight when he defeated Randy Couture at UFC 91 on November 15, 2008.

The former NCAA Division 1 heavyweight wrestling champion lost the UFC title to Cain Velasquez at at UFC 121 on October 23, 2010.

Lesnar was battling diverticulitis, a disease of the digestive tract, during that time. He underwent 2 surgeries. The second one in 2011 required a foot of his colon be removed.

"I felt robbed by diverticulitis. I felt robbed by being sick. I was feeling good and it took me a couple years to start feeling good. I'm at home, I'm working out, my life is great, everything's in tune, my contract's coming to an end with WWE, hey it's been a great time but something's missing," he told "Stone Cold" Steve Austin during an October 2015 podcast. "I was up front and totally honest with the company and told them I'm really thinking of pursuing getting back in the Octagon."

The 38-year old has an MMA record of 5-3.
