An Alexis retractor, or AWR, which can measure 17 centimeters (6 inches) in diameter, was left inside the mother’s body following the birth of her baby at Auckland City Hospital in 2020.
The AWR is a retractable cylindrical device with a translucent film used to draw back the edges of a wound during surgery.
The woman suffered months of chronic pain & went for several checkups to find out what was wrong, including X-rays that showed no sign of the device. The pain got so severe that she visited the hospital’s emergency department & the device was discovered on an abdominal CT scan & removed immediately in 2021.
New Zealand’s Health & Disability Commissioner, Morag McDowell, found Te Whatu Ora Auckland – the Auckland District Health Board – in breach of the code of patient rights, in a report released on Monday.
During the operation in 2020, a count of all surgical instruments used in the procedure did not include the AWR, the commission report found. This was possibly “due to the fact that the Alexis Retractor doesn’t go into the wound completely as half of the retractor needs to remain outside the patient & so it would not be at risk of being retained,” a nurse told the commission.
The case has also been referred to the director of proceedings, an official who will determine whether any further action should be taken.
“We would like to assure the public that incidents like these are extremely rare, and we remain confident in the quality of our surgical and maternity care,” read a statement from Te Toka Tumai Auckland

HOOVER, Ala. (AP) — A 25-year-old Alabama woman returned home late Saturday after being the focus of a 2-day search by police & family members who reported her missing after she stopped to check on a child who was walking along a highway.
Police said Carlethia “Carlee” Nichole Russell had returned to the home she shares with her parents in Hoover Saturday night.
Hoover Police Chief Nick Derzis said Russell arrived home alone & was brought to a hospital for evaluation.
Russell’s whereabouts were not immediately clear since around 10:45 p.m. Thursday when she called 911 & a family member to say she saw a young child walking on the side of Interstate 459.
Police found Russell’s car & her cellphone but were unable to find her or a child in the area.
Hoover Police Lt. Daniel Lowe said the family member on the phone with Russell lost contact with her even though the line remained open. A single witness reported possibly seeing a gray vehicle and a man standing outside of Carlee’s vehicle.
Talitha Russell said her daughter was headed home in the community about 10 miles south of Birmingham after leaving work & stopping to get food. She was on the phone with her brother’s girlfriend when she said she saw a child on the roadside.

The 18-year-old girl who went missing from Arizona in 2019 & re-emerged in Montana this week allegedly fought with the man she was living with & threatened to “go back” — just 1 day before she turned herself in at a police station.
Neighbors told The Post they heard a holed-up Alicia Navarro Saturday shouting with an unidentified man inside their apartment building in Havre.
“I was here the other day and I heard them yelling. She did say, ‘I will go back.’ But that’s all I heard,” neighbor Garrett Smith said.
Navarro sauntered into a local police precinct the following day to request that she be taken off the missing person’s list so she could get a driver’s license & could begin living a “normal life.”
She had been missing since police said she “willfully left her home” in Glendale, Arizona home in 2019.
It’s not clear how long Navarro has been living in the Havre apartment, but Smith said she & a man in his 20s have been residents since he moved in about a year ago.
He said that the teenager appeared scared, had a “scratchy voice” & had noticeably insufficient dental hygiene.
Police said Friday that a man was detained & questioned in connection to Navarro’s disappearance, though it is unclear whether it was the same individual she had shared the Havre apartment with.
Navarro fled her family home just days before her 15th birthday, leaving behind a note that read: “I ran away. I will be back. I swear. I’m sorry.”
A missing person report from years prior described Navarro as autistic but high-functioning.
Navarro is still being considered a victim, police previously said.
Navarro told police she was not being held against her will & could come & go freely, and said she had not been hurt. Police have said she does not face criminal charges.

Takeoff's mother Titania Davenport has filed a wrongful death lawsuit against the Houston venue where the Migos rapper was shot & killed on November 1.
According to the court docs, obtained by TMZ Hip Hop, Davenport claims the companies behind 810 Billiards & Bowling failed to take appropriate security measures to protect guests at the venue ... including her son, Takeoff.
The suit, filed Wednesday in Harris County, Texas also cites the lack of security prechecks while knowing Takeoff's celebrity would draw a huge crowd.
Patrick Xavier Clark, the man police say fired the fatal shot that killed the 28-year-old, isn't named in the suit ... but Davenport alleges 810 should have known there was a risk of violence based on past incidents on the premises and the crime in the surrounding area.
Takeoff had a huge memorial service inside State Farm Arena in his hometown Atlanta & the suit is seeking over a million dollars in damages to cover his funeral expenses, mental anguish & more.

BOGOTA, Colombia (AP) — 4 Indigenous children survived an Amazon plane crash that killed 3 adults & then braved the jungle for 40 days before being found alive by Colombian soldiers, bringing a happy ending to a search-and-rescue saga that captivated a nation & forced the usually opposing military & Indigenous people to work together.
Cassava flour & some familiarity with the rainforest’s fruits were key to the children’s extraordinary survival in an area where snakes, mosquitoes & other animals abound. The members of the Huitoto people, aged 13, 9 & 4 years & 11 months, are expected to remain for a minimum of 2 weeks at a hospital receiving treatment after their rescue Friday.
The children were being rehydrated & cannot eat food yet.
“But in general, the condition of the children is acceptable,” Defense Minister Iván Velásquez said. They were travelling with their mother from Araracuara to San Jose del Guaviare when the plane crashed on May 1.
The Cessna single-engine propeller plane was carrying 3 adults & the 4 children when the pilot declared an emergency due to an engine failure. The small aircraft fell off the radar a short time later & a search for survivors began.
“When the plane crashed, they took out (of the wreckage) a fariña & with that, they survived,” the children’s uncle said. Fariña is a cassava flour that people eat in the Amazon region.
“After the fariña ran out, they began to eat seeds.”
Timing was in the children’s favor. They were also able to eat fruit because “the jungle was in harvest.”
“The minors were already very weak,” Astrid Cáceres, head of the Colombian Institute of Family Welfare said. “And surely their strength was only enough to breathe or reach a small fruit to feed themselves or drink a drop of water in the jungle.”

LAKE BARRINGTON, Ill. (AP) — A suburban Chicago man is facing firearm charges after he told officers he accidentally shot himself in the leg while dreaming that an intruder was breaking into his home.
The Lake Barrington man was charged about 2 months after Lake County Sheriff’s deputies were called to his home on April 10 on a report about a person with a gunshot wound.
Deputies who found the 62-year-old man with a gunshot wound to 1 of his legs applied a tourniquet to the limb because he “was losing a significant amount of blood” before being hospitalized.
The man told investigators he “had a dream that someone was breaking into his home” & during that dream, he “retrieved his .357 Magnum revolver & shot at who he believed was the intruder.”
“When he fired, he shot himself & apparently woke up from the dream,” police said Tuesday.
Investigators determined there was no burglary attempt at the man’s home. But they found that his state firearm owner’s identification card had been revoked & he was still in possession of a firearm.
A warrant was issued June 9 for the man on charges of possession of a firearm without a valid FOID card & reckless discharge of a firearm, both felonies.
He was arrested Monday & released after posting bond & is scheduled for a June 29 court status hearing.

MEXICO CITY (AP) — A city councilor from the Mexican border city of Reynosa has been arrested at a checkpoint in Texas after U.S. border patrol agents found about 93 pounds (42 kilograms) of cocaine in her car, prosecutors said.
On Wednesday, Denisse Ahumada was still listed as one of about 20 members of the Reynosa city council.
Court records show she was arrested on June 10 at an inland security checkpoint in Falfurrias, Texas after an x-ray inspection of her SUV showed anomalies in the door panels & the seats. Border Patrol agents found 42 packages wrapped in duct tape & foil, each containing about a kilogram of cocaine.
Ahumada told agents she was supposed to have delivered the drugs to San Antonio, Texas. She also told agents she had carried such shipments before.
Local media reported Ahumada was part of the conservative opposition National Action Party, or PAN. But in a statement Tuesday, the PAN office in the northern border state of Tamaulipas denied she was a member or supporter of the party, and condemned drug smuggling.
State party leader Luis René Cantú said Ahumada had run for office on the Green party ticket and later asked to join the PAN, but later lost contact with the party.
Reynosa is a violent border city across from McAllen, Texas that has long been dominated by factions of the Gulf cartel.

SALT LAKE CITY (AP) — After her husband died last year, she wrote a children’s book on grief. Now she’s charged with his murder.
Kouri Richins was arrested on Monday in Utah & is accused in charging documents of poisoning her husband with a lethal dose of fentanyl at their home in Kamas.
Prosecutors allege that Richins called authorities in the middle of the night in March 2022 to report that her husband, Eric Richins, was “cold to the touch.” The mother of 3 told officers that she had made her husband a mixed vodka drink to celebrate him selling a home & then went to soothe 1 of their children to sleep in their bedroom. She later returned&d upon finding her husband unresponsive, called 911.
A medical examiner later found 5 times the lethal dosage of fentanyl in his system.
In addition to the murder charge, Richins also faces charges involving the alleged possession of GHB — a narcolepsy drug.
The charges — which are based on officers’ interactions with Richins that night & the account of an “unnamed acquaintance” who claims to have sold her the fentanyl — come 2 months after Richins appeared on local television to promote “Are you with me?” a picture book she wrote to help children cope after the death of a loved one.
For a segment entitled “Good Things Utah,” Richins called her husband’s death unexpected & described how it sent her & her 3 boys reeling.
“It’s — you know — explaining to my kid just because he’s not present here with us physically, doesn’t mean his presence isn’t here with us,” she told the anchors, who commended her for being an amazing mother.

An Idaho man was arrested after allegedly murdering the man who was the main suspect in his mom's murder.
Raul Alexander Cuevas, 31, was taken into custody last Wednesday, a day after his 52-year-old mother, Michelle E. Luna, was found mortally wounded with multiple stab wounds in Nampa.
Authorities later identified Jesus R. Urrutia, 39, as a suspect in Luna’s killing & he was charged with first-degree murder.
The Ada County Sheriff’s Office later reported that at approximately 7:03 a.m. the next day, there was a call of another stabbing victim, believed to be Urrutia, found at a gas station.
When Nampa police responded to the call they reportedly found Urrutia “slumped over in the car, which was running but in park.”
Police found Cuevas 20 miles away from the gas station & he was subsequently arrested for allegedly murdering Urrutia in what police believe was an act of revenge for Luna’s killing.
The Ada County Coroner’s Office ruled that Urrutia’s death was a homicide after he suffered from a “sharp force injury wound to the chest."
Cuevas was charged with first-degree murder, though authorities have not officially determined a motive behind Urrutia’s death & said they are “still figuring out why Cuevas fatally stabbed Urrutia.”

WINCHESTER, Ohio (AP) — 7 law enforcement officers have sued rap artist Afroman, accusing him of improperly using footage from a police raid on his Ohio home last year in his music videos.
4 deputies, 2 sergeants & a detective with the Adams County Sheriff’s Office brought the suit earlier this month, claiming invasion of privacy. Other law enforcement officers who were involved in the raid are not named as plaintiffs.
The plaintiffs say the rapper, whose real name is Joseph Foreman, took footage of their faces obtained during the August 2022 raid & used it in music videos & social media posts without their consent. They say that has caused them “emotional distress, embarrassment, ridicule, loss of reputation & humiliation.”
The plaintiffs are seeking all of Foreman’s profits from his use of their personas. That includes proceeds from the songs, music videos & live event tickets, as well as the promotion of Foreman’s Afroman brand, under which he sells beer, marijuana, T-shirts & other merchandise. They also seek a court injunction to take down all videos & posts containing their personas.
Law enforcement officers were acting on a warrant that stated probable cause existed that drugs & drug paraphernalia would be found on Foreman’s property & that trafficking & kidnapping had taken place there. Those suspicions turned out to be unfounded, though & the raid failed to turn up probative criminal evidence. No charges were ever filed.
When cash seized during the raid was returned to Foreman, it appeared that hundreds of dollars were missing. A subsequent review by the state Bureau of Criminal Investigation determined that deputies had miscounted the amount seized during the raid itself.

A small Tennessee police station has been rocked by allegations of wild sexual misconduct, after a married female officer allegedly had steamy romps with 6 male officers, including illicit on-duty liaisons.
Officer Maegan Hall & her fellow law officers allegedly engaged in wild sexcapades that included sending dirty pictures, taking her top off at a “Girls Gone Wild” hot tub party & even having oral sex with 6 officers at the La Vergne, Tenn., police station.
The steamy shenanigans — in which Hall allegedly bragged about the size of 1 partner’s genitals & claimed to be in an “open marriage” — not only led Hall to be canned, but led to the firings of Sgt. Lewis Powell, Officer Juan Lugo, Sgt. Ty McGowan & Detective Seneca Shields.
2 other officers who allegedly had romps with Hall — Patrick Magliocco & Larry Holladay kept their jobs but wound up suspended.
The extracurricular trysts took place at hotels & parties at other officers’ houses & on a boat, while Hall was also accused of performing oral sex on Powell & Shields while on duty at the police station & police gym.
Officials began to look into the swinging personal lives of the officers when La Vergne Mayor Jason Cole received a tip that Hall was sleeping with several fellow cops & had a threesome with Magliocco & his wife, among other claims.
When asked about the tip, Magliocco admitted to having sex with Hall on “numerous occasions.”

A Los Angeles rapper has filed a $10 million claim, alleging L.A. County deputies unjustifiably threatened to shoot him while he was sitting inside his car on New Year’s Eve.
Bodycam footage shows a deputy pointing a gun at Feezy Lebron (née Darral Scott), as he was sitting inside his car at a Gardena parking lot.
Lebron said he was sitting in his car listening to music while awaiting a friend when LASD deputies pulled up & “began to harass” him.
Lebron’s attorney says the deputies opened Lebron’s door without warning, grabbed his arm & attempted to pull him out of the car without any explanation.
“If you take off in this car, I’m going to shoot you,” says the deputy in the video. “I’m going to make it super easy on you. You put this car in drive, you’re getting one right to the chest.”
“Without any justification, the deputy drew his firearm & pointed it directly at Claimant at point blank range, who had his hands up & was not moving,” the statement read. “Claimant was then forced out of his car, searched & tightly handcuffed, causing him physical injuries…He was detained in the back of a patrol vehicle for approximately 20-40 minutes while deputies searched his car without consent or lawful justification.”
Ultimately, Lebron was not arrested & was instead given a ticket for a missing license plate.
Lebron thought he would be “killed & would never see his children again,” said the statement. “[He] has been completely traumatized by the incident & sought emergency care several days later. He continues to suffer panic attacks, is unable to sleep & is afraid to drive.”
The L.A. Sheriff’s Department said the incident is under investigation.

Four suspects have been arrested for the involvement of the case of the Lousiana State University student who was hit and killed by a vehicle in Baton Rouge earlier this month.
According to East Baton Rouge Police Department, a 17-year-old juvenile and Kaivon Deondre Washington, 18, are being accused of third-degree rape.
Two other men, Everette Lee, 28, and Casen John Carver, 18, are being accused of being principles to the crime. This means that they were present during the incident but were not directly involved.
According to police, Madison Brooks, 19, was leaving a bar in Tigerland around 1 a.m. when she entered the vehicle of the four suspects.
Washington and the unnamed juvenile are accused of raping her in the rear seats, while the other two suspects were in the front seat. The four suspects then dropped her off at a random location which led to her being hit by a vehicle around 3 a.m. on Burbank Drive near Pelican Lakes Parkway.
Deputies say Brooks' toxicology reports show her blood alcohol content (BAC) was .319g%. According to the University of Notre Dame Research, the effects of a BAC between .350g% and .399g% are listed as alcohol poisoning. Tetrahydrocannabinol (THC) was also found in her system.

BIRMINGHAM, Ala. (AP) — Alabama Gov. Kay Ivey sought a pause in executions and ordered a “top-to-bottom” review of the state’s capital punishment system Monday after an unprecedented third failed lethal injection.
Ivey’s office issued a statement saying she had both asked Attorney General Steve Marshall to withdraw motions seeking execution dates for two inmates and requested that the Department of Corrections undertake a full review of the state’s execution process.
Ivey also requested that Marshall not seek additional execution dates for any other death row inmates until the review is complete.
The move followed the uncompleted execution Thursday of Kenneth Eugene Smith, which was the state’s second such instance of being unable to put an inmate to death in the past two months and its third since 2018. The state completed an execution in July, but only after a three-hour delay caused at least partly by the same problem with starting an IV line.
Denying that prison officials or law enforcement are to blame for the problems, Ivey said “legal tactics and criminals hijacking the system are at play here.”
“For the sake of the victims and their families, we’ve got to get this right,” she said.

LOS ANGELES (AP) — A judge declared a mistrial Wednesday at the rape trial of “That ’70s Show” actor Danny Masterson after jurors, who were leaning strongly toward acquitting him, deadlocked following the monthlong trial in which the Church of Scientology played a supporting role.
Prosecutors said Masterson raped 3 women, including a former girlfriend, in his Hollywood Hills home between 2001 & 2003 & the church dissuaded them from going public for years. Masterson, 46, pleaded not guilty & his lawyer said the acts were all consensual.
“I find the jurors hopelessly deadlocked,” Judge Charlaine Olmedo declared after the jury foreman said there was nothing the court could do to move them closer to reaching a unanimous decision. She set a March date for a retrial in Los Angeles Superior Court.
Olmedo had ordered jurors to take Thanksgiving week off & keep deliberating after they said on Nov. 18 that they could not reach a consensus. A jury of 6 women & 6 men began deliberations anew Monday after alternates replaced 2 jurors who were diagnosed with COVID-19 over the break.
Jurors voted 7 times Tuesday & Wednesday without being able to reach consensus on any of the three counts, the foreman said. Two jurors voted for conviction on the first count, & voted for conviction on the second count & 5 voted to convict on the third count.
The result was a serious setback for prosecutors & for the 3 women who said they were seeking long overdue justice & provided emotional & graphic testimony over several days.
All 3 women were members of the church at the time, and Masterson remains one.
2 of the women & the husband of 1 are suing Masterson, the Church of Scientology, its leader David Miscavige & others for allegedly stalking, harassing & intimidating them after they sought to expose Masterson.
